Default Credit fraud question (mortgage)?

HI I have a questioned about a credit issue. I looked at my credit report last year and noticed I had a mortgage on there from 2002 which I never had since I don't own a home. I disputed and said I don't own a home I never received the report or outcome of the dispute cause they have the wrong address on file. I checked my credit report this year and the home loan is still on there however the last it was reported was in 2004 and states I owe nothing. I have a feeling I know who did this but the loan isn't in my name anymore. How can I find out the address to where the home is? And what can I do about the person who used my SSN# to get it? Im assuming they must have refinanced in 2004 as that's the last date it was reported. Any help would be great I looked up the mortgage company and there no longer in business but switched to a new company which did not have any record of me on the automated system.I don't think it's completely cold just yet as I disputed it and it wasn't taken off my credit report. Doesnt that mean the credit burreaus were able to verify it as a legitamte debt? I haven't spoken to a live person at the loan company yet but since they bought out the other company they should have my records right? So confusing to try and find who used your social.
